Established in 2021, our Public Health Bilingual Community Health Workers (CHWs) provide equitable access and resources to individuals in our communities that are most at-risk.
What is a Community Health Worker (CHW)?
- A trusted member of the community
- Demonstrates working knowledge of the community and people served
- Promotes equity in health and access to health and prevention resources
How do CHWs provide services to you?
- Act as a link between various providers and their patients
- Help guide community members through the health system and find resources for clients (housing, food, supplies, and more)
- Provide support during pregnancy and help patients with chronic disease and more!
Our CHWs also support the Sheboygan County Division of Public Health's Families + Neighbors Together program, an initiative focused on prevention and early intervention to support families and communities.
